Job summary

Sapphire Health Services in Tigard, Oregon is seeking an Infection Prevention / Admissions RN to lead the community's Infection Prevention and Control Program and to coordinate safe, timely admissions. The role collaborates with nursing leadership, hospitals, and families to ensure clinical information, medications, and treatments are properly addressed during transitions.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification as required by the community.
• Infection prevention certification or specialized training preferred.
• Previous nursing experience in skilled nursing/long-term care preferred.
• Knowledge of long-term care regulations and infection prevention standards preferred.

Responsibilities

• Coordinate and maintain the Infection Prevention and Control Program per applicable regulations and policies.
• Monitor infection prevention practices including hand hygiene, PPE, and isolation procedures.
• Coordinate admissions and transitions of care with nursing leadership, hospitals, and referral sources.
• Provide infection prevention education and participate in rounds, audits, and survey readiness.
• Review hospital and referral documentation to identify resident needs and ensure complete documentation.
• Coordinate medications, treatments, supplies, and equipment for admissions.
• Communicate resident needs and hand off to the care team to ensure continuity of care.
• Follow up on outstanding admission needs and report significant clinical concerns.

JOB SUMMARY:

The Infection Prevention / Admissions RN is responsible for coordinating the community’s Infection Prevention and Control Program while supporting the safe, efficient, and clinically appropriate admission of residents into the community.

This position provides clinical oversight of infection prevention practices, conducts infection surveillance, monitors trends, supports outbreak response, provides staff education, and promotes compliance with applicable infection prevention standards and regulatory requirements.

In the admissions function, the RN collaborates with nursing leadership, admissions personnel, hospitals, referral sources, residents, and families to coordinate new admissions and ensure clinical information, physician orders, medications, treatments, equipment, and other resident needs are appropriately communicated and addressed during the transition into the community.

The Infection Prevention / Admissions RN serves as a clinical resource to the interdisciplinary team and supports resident safety, continuity of care, regulatory compliance, and a positive admission experience.

RESPONSIBILITIES:

• Coordinate and maintain the community’s Infection Prevention and Control Program in accordance with applicable federal, state, and local regulations and organizational policies.
• Coordinate and maintain the community’s Infection Prevention and Control Program in accordance with applicable regulations, standards, and organizational policies, including infection surveillance, tracking, documentation, and identification of trends.
• Monitor infection prevention practices throughout the community, including hand hygiene, PPE use, standard and transmission-based precautions, isolation procedures, and other infection control measures.
• Identify and respond to potential infections and outbreaks, collaborating with nursing leadership, providers, public health agencies, and the interdisciplinary team to coordinate testing, precautions, monitoring, and appropriate interventions.
• Provide infection prevention education and coaching to staff and participate in infection prevention rounds, audits, policy implementation, QAPI activities, and survey readiness.
• Support the community’s Antibiotic Stewardship Program by monitoring antibiotic utilization and collaborating with providers, pharmacy, nursing leadership, and the interdisciplinary team as appropriate.
• Coordinate with Admissions and nursing leadership to support safe, timely, and clinically appropriate resident admissions and transitions of care.
• Review hospital and referral documentation, including diagnoses, physician orders, medications, treatments, laboratory results, and other clinical information to identify resident needs and ensure documentation is complete and accurate.
• Complete or assist with nursing admission assessments, medication reconciliation, and required clinical documentation, obtaining clarification or additional information from providers, hospitals, pharmacies, or referral sources as needed.
• Coordinate medications, treatments, supplies, durable medical equipment, oxygen, specialty services, infection precautions, and other identified needs to ensure the community is prepared for the resident’s admission.
• Communicate resident needs, risks, precautions, and care requirements to nursing staff and the interdisciplinary team and provide appropriate clinical handoff to promote continuity of care.
• Follow up on outstanding admission needs and communicate significant clinical concerns or changes in condition to the provider and nursing leadership as appropriate.
• Perform other duties as assigned.

REQUIREMENTS:

• Current, active, and unrestricted Registered Nurse (RN) license in the state of practice.
• Current CPR/BLS certification as required by the community.
• Infection prevention certification or specialized training in long-term care infection prevention preferred.
• Previous nursing experience in skilled nursing, long-term care, Post-Acute care, or a related healthcare setting preferred.
• Previous experience with infection prevention, infection surveillance, quality assurance, staff education, or clinical leadership preferred.
• Knowledge of long-term care regulations and infection prevention standards is strongly preferred.
