Job summary

Trilogy Health Services is seeking an Infection Control and Wound Nurse (LPN, RN) to support the DON and ADON by leading the wound care program and outlining treatment plans. Responsibilities include monitoring Braden scores, weekly wound measurements, and ensuring comprehensive infection control measures are in place.

The role requires an active nursing license, CPR certification, and preferred WCC or Infection Preventionist certification.

Qualifications

• Must hold a current, valid state LPN or RN license.
• Wound Care Certification (WCC) preferred or willingness to complete training.
• Infection Preventionist certification or willingness to complete training.

Responsibilities

• Oversee wound care and infection control to support proper intervention plans.
• Develop and maintain an active Infection Control program with antimicrobial stewardship.
• Ensure orders, care plans, and CNA records are accurately documented.
• Participate in survey readiness and campus preparedness efforts.
• Make daily rounds to ensure nursing staff adhere to infection control standards.
