Trilogy Health Services, LLC is seeking an Infection Control and Wound Nurse (LPN, RN) to support the DON and ADON in overseeing the wound care program and treatment plans at The Oaks at Bethesda in Zanesville, OH. The role emphasizes infection control, wound assessment, and staff oversight.

The position requires an active LPN/RN license, CPR, and preferred WCC or Infection Preventionist credentials. Compensation is based on license, certification, and experience, with comprehensive Thrive

Qualifications

• Current, valid state LPN or RN license.
• Wound Care Certification preferred or willingness to complete.
• Infection Preventionist certification or willingness to complete.
• Current CPR certification required.

Responsibilities

• Monitor Braden Scale results and implement prevention for high-risk residents.
• Develop and maintain an Infection Control program with binder and antimicrobial stewardship.
• Ensure orders and care plans are in medical records; update CNA sheets.
• Participate in survey and mock-survey campus preparedness.
• Make daily rounds to ensure nursing staff follow standards.
• Review fridge temps, crash cart audits, med rooms and HIPAA compliance; report findings.
• Provide patient care as needed and participate in on-call rotation.
