Job summary

Venture Community Services seeks an Individual Supports Caseworker to assess referrals and teach community living skills, enabling individuals to remain in their homes and thrive. You will deliver direct training and coordinate with the Director of Day and Community Services to ensure program goals are met.

The role requires timely, accurate documentation, collaboration in multidisciplinary teams, and a commitment to ongoing education and professional development.

Qualifications

• Bachelor's Degree as listed or 60 credits toward degree with 3 years' experience in special education.
• Must be 21+ with High School Diploma or GED, valid driver's license, and daily car on site.
• Fluent English: speaking, reading and writing.

Responsibilities

• Conduct home visits to assess referrals and implement Individual Skills Support strategies weekly.
• Maintain daily activity statistics and required documentation.
• Represent the department in multi-disciplinary meetings for planning and review of Individual Support Plans.
• Provide direct individual training in community living skills as defined by program contract.
• Attend workshops and other continued education programs.
• Prepare reports and maintain confidentiality and budget records.
