Job summary

FOX Rehabilitation in Granby, CT is seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes and senior living communities. You will assess, plan, and deliver evidence-based treatment while maintaining autonomy and a flexible schedule.

Qualifications

• Valid Physical Therapy license in the state of practice (CT).
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and function.
• Document effectively in the EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and fitness staff.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access
