Job summary
Height Talent Solutions is seeking a dedicated Physical Therapist for Home Health across Pennsylvania and Michigan. You will provide skilled PT services in patients' homes, collaborating with a clinical team to deliver high-quality outcomes.
This full-time role offers independent work, a flexible schedule, and a pay-per-visit compensation structure with travel to local patient homes. The successful candidate will have a PT license in PA or MI and at least 1 year of healthcare experience.
Qualifications
- Licensed PT in PA or MI per state requirements.
- Bachelor's or Master's degree in Physical Therapy from an accredited program.
- Minimum 1 year healthcare experience; new grads may be considered.
- Valid driver's license and reliable transportation.
- Experience with EMR documentation; HomeCare HomeBase a plus.
Responsibilities
- Deliver skilled physical therapy services in residential home settings.
- Develop and implement individualized care plans with the care team.
- Complete clinical documentation accurately and on time using EMR.
- Travel locally to patient homes; approximately 20% driving.
- Manage your own schedule and caseload in assigned territory.
