Job summary

Cornerstone VNA is seeking a compassionate Physical Therapist (PT) to join our Home Care Rehabilitation team in Rochester, NH. Provide one-on-one, in-home care to help patients regain mobility and independence with individualized plans and regular progress updates.

Ideal candidates hold NH and ME PT licenses, have 2+ years of experience, and are comfortable with a high travel requirement. Full-time or part-time roles available with mileage reimbursement and generous benefits.

Qualifications

• Current Physical Therapist license in New Hampshire and Maine.
• 2+ years of experience preferred.
• Home care experience preferred (minimum 1 year).
• Strong clinical judgment and documentation skills.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide skilled PT services in the home to improve mobility and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate patients and caregivers on plans, exercises, safety, and progress strategies.
• Complete timely documentation in the EMR and coordinate care.
