Job summary

Qualifications

• Active California Physical Therapy license or eligibility.
• Ability to work independently with clinical autonomy.

Responsibilities

• Provide Physical Therapy services consistent with licensure and facility policies.
• Perform evaluations and develop treatment plans for referred patients.
• Document progress notes and communication with medical staff daily.
• Available for telephone consultation while on duty.
• Maintain all charting and ensure timely documentation.
