Job summary

AvaMed Workforce is seeking a full-time Physical Therapist to support a California correctional facility in Vacaville, CA. The position offers 40 hours per week with a 4/10 schedule and a competitive pay rate of $85 per hour (1099).

Orientation is two weeks, and hours are Monday–Thursday 7:00am–3:00pm and Friday 7:00am–11:00am. Responsibilities include evaluating and treating patients per licensure, coordinating with medical staff, and maintaining charting and documentation.

Responsibilities

• Perform care per licensure scope and institution policies, aligned with evidence-based PT practices.
• Provide PT evaluations on an as-needed basis as scheduled by the Facility designee.
• Deliver PT services upon physician referral or direct access for existing diagnosed cases.
• Conduct initial assessments and develop treatment plans; communicate them to patient and staff.
• Complete reports, assessments, and progress notes in the chart using approved forms.
• Be available for telephone consultations while on duty.
• Maintain daily charting; delinquent charts may risk termination.
