Job summary

Walmart is seeking an Independent Optometrist leasing space inside Walmart stores to lead eye and vision care within the store environment. You will establish your practice days, hours of operation, and patient schedule, while retaining 100% of exam and professional fees.

You will determine your fee schedule and can opt into equipment, instrumentation, and support services where allowed by state regulations. This role emphasizes autonomy within a retail setting and requires appropriate licensure.

Qualifications

• OD degree required from an accredited optometry program.
• MD/DO ophthalmology degree accepted where applicable.
• Active state optometry license is mandatory.
• Active state ophthalmology license may be required for cross-coverage.
• General liability insurance certificate.
• Professional liability insurance certificate.
• W-9 form required for independent contractor arrangements.

Responsibilities

• Provide eye and vision care as an independent optometrist leasing space inside Walmart stores.
• Establish and manage your practice days, hours, and patient schedule.
• Set your own fee schedule and retain 100% of exam/professional fees.
• Coordinate with store operations and maintain compliant practice standards.

Doctor of Optometry (OD) from an accredited optometry school

Ophthalmologist (MD or DO) from an accredited school

Valid state optometry license

As an Independent Optometrist leasing space inside our Walmart stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being.

As an independent lease holder, you have the autonomy to:

• Establish your days of practice
• Establish your hours of operation
• Control your patient schedule
• Set your fee schedule
• Keep 100% of exam/professional fees

For a fair market value, you can opt into:

• Equipment/instrumentation
• Administrative and/or Opt-Tech support services (if state allows)

Minimum Qualifications for a lease agreement:

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9
