Job Title: Independent Optometrist - Walmart

Optional support services are available for a fair market value, including equipment/instrumentation and administrative/Opt-Tech support (if permitted by state).

Minimum Qualifications:

• Doctor of Optometry (OD) from an accredited school with a valid state license
• Ophthalmologist (MD or DO) from an accredited school with a valid state license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
