Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will lead patient care related to eye and vision health, as well as general well-being. You have the autonomy to:

1. Establish your practice days and hours
2. Control your patient schedule
3. Set your fee schedule
4. Keep 100% of exam and professional fees

Optional support services and equipment may be available for a fair market value, including:

• Equipment and instrumentation
• Administrative and Opt-Tech support (if permitted by state)

Minimum qualifications for a lease agreement include:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
