Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will play a key role in providing eye and vision care, as well as promoting overall health and well-being.

Your autonomy as a leaseholder includes:

• Establishing your practice days and hours
• Controlling your patient schedule
• Setting your fee schedule
• Retaining 100% of exam and professional fees

Optional services at fair market value include:

• Equipment and instrumentation
• Administrative and/or Opt-Tech support services (if permitted by state)

Minimum qualifications for a lease agreement include:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements:

• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form

Note: ME & VA license agreements are managed by Gumberg Asset Management Company.
