Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will lead in eye and vision care, as well as general health and well-being.

You will have the autonomy to:

1. Establish your practice days and hours
2. Control your patient schedule
3. Set your fee schedule
4. Keep 100% of exam/professional fees

Optional support services available at fair market value include:

1. Equipment and instrumentation
2. Administrative and/or Opt-Tech support (if permitted by state)

Minimum qualifications for lease agreement:

1. Doctor of Optometry (OD) from an accredited school
2. Valid state optometry license
3. Ophthalmologist (MD or DO) from an accredited school
4. Valid state ophthalmology license
5. General Liability Certificate
6. Professional Liability Insurance Certificate
7. W-9 form
