Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will take a leading role in patient care related to eye and vision health, as well as general well-being.

Your autonomy includes:

• Establishing your practice days and hours
• Controlling your patient schedule
• Setting your fee schedule
• Retaining 100% of exam and professional fees

Optional support services for fair market value include equipment/instrumentation and administrative or opt-tech support, if permitted by state regulations.

Minimum qualifications for lease agreement:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements:

• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
