Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will lead patient care related to eye and vision health, as well as general well-being. You will have the autonomy to:

1. Establish your practice days and hours
2. Control your patient schedule
3. Set your fees
4. Retain 100% of exam/professional fees

Optional support services available at fair market value include equipment/instrumentation and administrative or opt-tech support, where permitted by state law.

Minimum qualifications for lease agreement:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements:

• Ophthalmologist (MD or DO) with valid state license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
