Qualifications

• OD from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9.
• ME & VA license agreements are with Gumberg Asset Management Company.

Responsibilities

• Launch your eye care services in Walmart stores as an Independent Optometrist.
• Establish your days and hours of operation and maintain control over your schedule.
• Set your fee schedule and keep 100% of exam/professional fees.
• Provide eye and vision care and contribute to patients' general health and well-being.
• Access equipment/instrumentation and administrative/Opt-Tech support where allowed by state.
