Qualifications

• Doctor of Optometry OD from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD or DO from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate/Professional Liability Insurance Certificate.

Responsibilities

• Launch services in Sam's Club and lead patient care in eye and vision care.
• Establish practice schedule, control patient schedule, and set fee schedule.
• Option for advanced equipment and administrative support if state allows.
