Qualifications

• OD degree required and licensed to practice optometry in the state
• Valid professional liability and liability coverage certificates are required
• Lease agreement requires a valid W-9 and business setup
• Independent practice with autonomy over schedule and fees

Responsibilities

• Provide comprehensive eye and vision care within the Sam’s Club setting
• Establish practice days and hours, and schedule patients
• Set the fee schedule and manage patient flow
• Maintain professional standards and compliance within Walmart policies
