Qualifications

• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.

• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9.

Responsibilities

• Establish days of practice.
• Establish hours of operation.
• Control patient schedule.
• Set fee schedule.
• Keep 100% of exam/professional fees.
• Launch services in Walmart stores.
