Qualifications

• OD degree from an accredited optometry school.
• Valid state optometry license.
• Ophthalmology license (MD/DO) valid where practicing.
• General liability and professional liability certificates.
• W-9 form for independent lease arrangements.

Responsibilities

• Lead eye and vision care within the Sam's Club lease space.
• Set practice days, hours, and patient schedules.
• Establish your fee schedule and manage patient flow.
• Provide high quality patient care and maintain standards.
