Qualifications

Responsibilities

• Support individuals with daily routines, including meal preparation and household tasks.
• Provide medication management support, including reminders or monitoring self-administration as trained and authorized.
• Encourage independence while offering guidance and support as needed.
• Assist with community outings and transportation to activities of the individual’s choice.
• Complete daily notes and documentation in the agency’s online system accurately and on time.
• Monitor and document behaviors and progress according to individual support plans.
• Provide supervision and ensure safety within the home and community.
• Follow all agency policies, safety procedures, and regulatory requirements.
• Provide respectful, professional support that maintains dignity and boundaries.
