Job summary

Qualifications

• Valid Driver’s License and acceptable driving record.
• Reliability and willingness to learn new opportunities.
• Ability to work in residential/community settings.

Responsibilities

• Develop individualized, person-centered service plans and help implement them.
• Provide daily living support and help with personal goals.
• Assist with transportation and access to community resources.
• Promote community inclusion and meaningful relationships.
• Maintain safe, clean, and supportive environment; document accurately.
• Complete and maintain required documentation timely.
