• Valid Driver’s License and acceptable driving record required.

Responsibilities

• Assist in developing individualized, person-centered service plans and help implement them.
• Provide support with daily living activities, skill development and personal goal achievement.
• Assist individuals with transportation and community resources as needed.
• Promote community inclusion, relationship building, and personal growth.
• Maintain a safe, clean, and supportive environment and complete required documentation.
