AbilityPath is hiring Independent Living Skills Direct Support Staff in San Jose, CA. We support adults with diverse abilities to live independently through coaching, planning, and community involvement.

Roles include life-skills coaching, independent living goals, and person-centered planning with a focus on health, recreation, and job skills. Full-time and part-time shifts available, with competitive benefits and growth opportunities.

Qualifications

• 1+ years previous work or volunteer experience supporting adults with disabilities.
• Associate degree or working towards completion.

Responsabilités

• Life Skills Coaching: provide one-on-one coaching to develop essential life skills for independent living.
• Independent Living: collaborate with individuals and teams to set goals and develop strategies for independence.
• Person-Centered Planning: work with individuals to create and implement individualized support plans.
• Physical Health and Well-being: promote healthy lifestyles and wellness activities.
• Recreational and Social Activities: enable participation in social and community events.
• Job Skill Training: explore career interests and build pre-vocational skills.
• Celebrate Progress: document learning and progress in the case management system.
