Job summary

Concern Housing is seeking a Daily Living Skills Counselor to provide hands-on life skills training to clients in the supported housing program, helping them achieve independence and self-reliance.

You will work as part of the case management team, provide education across activities of daily living, and maintain documentation while supporting residents through goal planning and skill development.

Qualifications

• Three years' experience in a similar setting or with people with mental illness.
• Good verbal and written communication skills.
• Ability to make sound judgments regarding client care.

Responsibilities

• Provide education and skill-building across activities of daily living (hygiene, environment, laundry, travel, money management, cooking, nutrition, shopping, self-preservation, communication, community integration, wellness).
• Provide individual counseling and support for life skills development.
• Collaborate with the case management team and keep open communication on goals and assessments.
• Develop training tools with the case manager to address client needs.
• Maintain all documentation per agency policy and attend supervision meetings.
