Peace Living LLC in Antelope Valley seeks a compassionate Coordinated Family Support (CFS) Professional to assist adults with developmental disabilities in building independence and improving quality of life. The role involves helping with daily living, medical follow-through, and navigating community resources.

You will coordinate services, document progress, and advocate for consumers across various programs.

Qualifikationen

High school diploma or associate degree in human services.
At least 1 year of experience in the developmental disabilities service delivery system.
Knowledge of the Regional Center system; experience preferred.

Aufgaben

Support individuals in developing daily living, self-advocacy, and community skills.
Assist with medical follow-through and accounting of funds, filing receipts.
Help individuals access resources like SSI, Medi-Cal, CalFresh, IHSS, and public transportation.
Coordinate appointments, services and future planning based on individual needs and goals.
Maintain accurate service notes and progress documentation.
Perform other duties to advocate for the consumer served.
