Blueprints is hiring an Independent Living Caseworker to provide case management and life skills services for youth. The role includes in-home sessions, progress reporting, and coordinating with community resources. Strong collaboration with Children and Youth Services is required.

The position emphasizes building trust with youth, crisis intervention on an on-call rotation, and adherence to agency policies and regulations while representing the organization in the community.

Formación

• Bachelor Degree in social work or related field.
• Caseworker II & III: Bachelor degree in Social Work or a related field with 2+ years' experience working with children and families or Master's degree in Social Work or related field.

Responsabilidades

• Conduct individual in-home sessions and facilitate life skills training.
• Maintain accurate documentation and case records and develop transition plans and progress reports.

• Attend court hearings and interdisciplinary meetings; participate in crisis intervention via on-call rotation.

This position is responsible for providing comprehensive case management and life skills services to youth while ensuring compliance with agency policies, contractual obligations, and applicable regulations. Key responsibilities include conducting individual in-home sessions, facilitating life skills training, maintaining accurate documentation and case records, developing transition plans and progress reports, and coordinating services with Children and Youth Services and other community providers. The role requires building positive relationships with youth, connecting participants to appropriate community resources, attending court hearings and interdisciplinary meetings, and participating in crisis intervention through an on-call rotation. Additionally, the position requires regular attendance, participation in required trainings, conferences, and retreats, and ongoing representation of the agency's mission and values within the community.
