Job summary

Venture Community Services seeks a Supports Case Worker in Worcester, MA. This full-time role provides person-centered case management for individuals in Supported Collaborative Living and Independent Living Supports. You’ll coach clients toward independence, coordinate services, and ensure documentation aligns with plans.

You will support budgeting, transportation, appointments, and access to community resources. A valid driver’s license and English proficiency are required.

Qualifications

• Bachelor's degree preferred in social work, psychology, or related field.
• High school diploma or GED required.
• Minimum age 21 and valid driver's license.
• Experience in direct support, case management, or independent living support preferred.
• Ability to communicate effectively in English, both written and oral.

Responsibilities

• Provide person-centered case management and coordination for SCL/ILS participants.
• Coach budgeting, bill payment, banking, and money-management skills.
• Assist with independent living, routines, housing upkeep, and safety.
• Help access medical, dental, behavioral health care and coordinate appointments.
• Oversee medication systems and ensure documentation and plans are up-to-date.
• Document services and progress in accordance with agency requirements.
