Job summary

Responsibilities

• Support individuals with activities of daily living, personal needs, and community involvement.

• Oversee financial records and money-management plans.
• Assist with meal planning, preparation, and grocery shopping.
• Plan and coordinate healthcare, vocational, and social activities.
• Complete monthly financial packets according to established procedures and timelines.
• Provide case management support to maintain a safe and healthy home environment.
• Coordinate transportation to community resources, medical appointments, job interviews, and other activities.
• Provide non-violent crisis intervention and support as needed.

You’ll work one-on-one with individuals living independently in the community, helping them navigate everyday life while encouraging independence, confidence, and personal choice. From planning appointments and managing finances to grocery shopping, community activities, and everything in between.

You’ll become an important and trusted part of the individuals’ lives while working alongside a supportive team of coworkers, clinical professionals, and supervisors.

Compensation: $20.96–$22.65 per hour, based on experience
Location: Yarmouth Port, MA
Schedule: Monday–Friday, 8:00am-3:00pm

What We're Looking For
