Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team in Vermont. You will provide skilled nursing and rehabilitative care to clients in their homes, helping them remain independent and comfortable at home.

The ideal candidate will have a Vermont nursing license, at least one year of adult/geriatric clinical experience, and comfort making real-time clinical decisions with strong assessment and documentation skills.

Qualifications

• Current Vermont nursing license in good standing.
• At least 1 year of adult/geriatric clinical experience in acute care, home health, or similar.
• Experience with wound care, IV therapy, and post-surgical care preferred.

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition.
• Triage and manage urgent clinical situations in the home setting.
• Conduct medication reconciliation, teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.
