Job summary

BAYADA Home Health Care is seeking a Registered Nurse to join our home health visit team as a Full Time RN. You will provide skilled nursing and rehabilitation in patients' homes, delivering high-quality care with compassion and reliability.

In this role you will perform comprehensive assessments, manage urgent issues, and ensure accurate medication reconciliation and wound documentation. A PA nursing license and at least one year of relevant experience are required.

Qualifications

• Active nursing license in good standing in Pennsylvania.
• Graduation from an accredited nursing program with transcript or diploma.
• Minimum one year of hands-on medical experience with adults/geriatric clients in various settings.

Responsibilities

• Perform comprehensive head-to-toe assessments and identify condition changes.
• Triage and independently manage urgent clinical situations in the home.
• Coordinate medication reconciliation, teaching, and safety monitoring.
• Document wound care, assessments, and progress in the chart.
