Qualifications

• Current nursing license in good standing in the state.
• Graduation from an accredited and approved nursing program.
• At least one year of current or recent hands-on medical experience with adult/geriatric populations in acute care, ER, home health, hospice, ICU, med-surg, sub-acute, or rehab settings.

Responsibilities

• Use head-to-toe assessment skills to identify changes in patient condition.
• Provide care in home health with acute and post-acute needs.
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care, and documentation.
