Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist Assistant to deliver home health services in the Santa Isabel area. This independent contractor role focuses on assessing patients, planning treatments, and supporting movement and functional goals for adults of all ages.

You will work autonomously with clinical support as needed, maintain patient records with discretion, and coordinate care with the supervising therapist. Flexibility and a patient-centered approach are essential.

Qualifications

• Must hold a Physical Therapist license and state registration.
• Experience in rehabilitation and home health settings.
• Bilingual a plus.

• Assess and evaluate patient care needs to promote movement and function.
• Develop and implement treatment plans and monitor progress.
• Document and maintain patient records with accuracy.
