Klarus Home Health in San Antonio, TX is seeking a dedicated Physical Therapist to join our in-home care team, serving patients across ages.

You will develop and implement individualized therapy plans, evaluate progress, coordinate with other professionals, and educate patients and families on treatment, home exercises, and prevention. Qualifications

• Education: B.S. in Physical Therapy from an APTA-approved program.
• Licensure: Current State License or PTLC-compliant multi-state license where applicable.
• Experience: 1+ year in acute care, rehab, or home health preferred.

Responsabilités

• Develop and implement individualized physical therapy treatment plans.
• Evaluate and document patient progress; adjust treatments as necessary.
• Coordinate care with other healthcare professionals for comprehensive treatment.
• Educate patients and families on treatment plans, home exercises, and preventive care.
• Maintain accurate patient records in compliance with standards.
• Utilize durable medical equipment and provide training to patients.
• Ensure patient safety and comfort during therapy sessions.
• Engage in ongoing professional development and keep up with best practices.
