Quality Home Health, Hospice & Palliative Care in Northern Utah is seeking a compassionate Physical Therapist to provide in-home care across Brigham City, Ogden, Clearfield, Layton, and surrounding communities. This pay-per-visit position offers mileage reimbursement and flexible scheduling, empowering you to deliver personalized care and help patients regain mobility and independence.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, unrestricted Utah Physical Therapist license.
• Home health experience preferred but not required.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently while maintaining effective communication with the clinical team.
• Compassionate, patient-centered approach to care.
• Reliable transportation and willingness to travel to patients' homes throughout the North Region.

Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.
• Develop and implement individualized treatment plans.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, and functional independence.
• Monitor patient progress and adjust treatment plans as appropriate.
• Educate patients and caregivers on exercises, safety, mobility, and home programs.
• Collaborate with physicians and interdisciplinary team members to coordinate high-quality patient care.
• Complete accurate and timely clinical documentation in accordance with agency policies and applicable regulations.
• Complete required home health assessments and reassessments, including applicable OASIS documentation.
• Participate in care coordination and quality improvement activities as needed.
