Job summary

Choice Health at Home is seeking a Full-Time Home Health Physical Therapist to serve patients in the Denver area. You will provide one-on-one in-home therapy to improve mobility, strength, and independence, delivering care in patients' homes with the flexibility of a home health role.

The role requires graduate of an accredited PT program, CPR, a valid driver’s license, and strong communication skills. Home health experience and HCHB experience are a plus, and a competitive benefits package is

Qualifications

• Graduate of an accredited Physical Therapy program.
• CPR certification.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.
• Homecare Homebase (HCHB) experience is a plus.
• Strong communication, organization, and time-management skills.

Responsibilities

• Perform comprehensive initial evaluations and identify patient needs, functional limitations, and appropriate therapy goals.
• Develop and implement individualized plans of care designed to improve mobility, strength, balance, and overall function.
• Provide skilled physical therapy interventions in patients’ homes.
• Evaluate patient progress and adjust treatment plans to achieve the best possible outcomes.
• Educate patients and caregivers on exercises, mobility techniques, fall prevention, safety, and continued progress.
• Collaborate with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary care team.
• Maintain timely and accurate clinical documentation in accordance with company and regulatory guidelines.
• Follow infection-control and universal-precaution standards.
• Help patients remain safe, independent, and successful in the place they call home.
