Job summary

ConnectedHealth in Arcata, CA is seeking a Full-time Family Practice Nurse Practitioner to provide comprehensive primary care to patients of all ages. You will diagnose, treat, order tests, and prescribe in California while collaborating with a multidisciplinary team.

Document encounters in the EMR, maintain regulatory compliance, and participate in quality improvement. A patient-centered approach and strong communication are essential for success in this role.

Qualifications

• Current, unrestricted California RN license.
• Current, unrestricted California NP license.
• Graduate of an approved NP program meeting California BRN standards.
• Furnishing credentials permitting medications/devices in California.
• Current, unrestricted DEA registration for California.
• BLS and ACLS certifications.
• Minimum of two years NP experience.
• Ability to work independently while collaborating with a healthcare team.
• Strong communication and organizational skills.

Responsibilities

• Provide comprehensive primary care services to patients of all ages.
• Perform physical examinations and comprehensive patient assessments.
• Diagnose and treat acute and chronic medical conditions.
• Develop individualized treatment plans based on evidence-based practice.
• Order, interpret, and evaluate diagnostic tests and laboratory results.
• Prescribe medications and devices in accordance with California regulations.
• Provide preventive healthcare, wellness counseling, and patient education.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Document patient encounters within the electronic medical record.
• Maintain compliance with organizational policies and state/federal regulations.
• Participate in quality improvement initiatives and patient care programs.
• Promote a positive, patient-focused healthcare experience.
