Job summary

FeldCare Connects seeks a self-motivated Occupational Therapy Assistant to deliver premier home health care in the Vacaville area. The OTA will assess patient needs, create treatment plans, provide interventions, and monitor progress for adults of all ages.

Responsibilities include leading therapeutic activities, coordinating with the health care team, and educating patients and families while maintaining flexible scheduling typical for independent contractors.

Qualifications

• Active OTA license and state registration.
• Completion of an accredited OTA program.
• Ability and enthusiasm in working with all ages.

Responsibilities

• Assess patient care needs and determine functional status and ADLs.
• Develop and implement an OTA treatment plan and monitor progress.
• Lead patients in therapeutic activities and instruct care partners.
• Observe, record, and report treatment responses and changes.
