Job summary

FeldCare Connects is an app-based therapy network and home health provider currently seeking a self-motivated Certified Occupational Therapy Assistant to deliver high-quality home healthcare to patients of all ages in San Dimas.

The OTA will work as an independent contractor, following an established plan of care, performing interventions to enhance daily living activities, and coordinating with the health care team to optimize patient outcomes.

Qualifications

• Graduation from accredited Certified Occupational Therapy Assistant program.
• California Certified Occupational Therapy Assistant license.
• Ability and enthusiasm in working with all ages.
• Bilingual is a plus.

Responsibilities

• Provide patient care focusing on functionality status and activities of daily living.
• Perform interventions to improve wellbeing and safety, following the care plan.
• Observe, record, and report patient progress to the care team.
• Instruct patients, families, and caregivers on therapy activities they can participate in.
