Health Connect America is seeking an Independent Contractor-Family Support Services to enhance family functioning and child well‑being. You will coordinate with community providers, participate in case planning, and deliver hands-on support including supervision, parent aide services, and crisis intervention.

The role requires a Bachelor's in a human services field (or a HS diploma with 5 years of relevant experience), reliable transportation, a valid driver's license, and ability to pass

Formación

• Bachelor's degree in a human services-related field or equivalent experience.
• HS Diploma/GED with at least 5 years of verifiable field experience.
• Strong understanding of family systems, trauma-informed care, and community resources.
• Reliable transportation with valid license and proof of insurance.
• Willingness to complete background checks and contractor onboarding.

Responsabilidades

• Coordinate with community providers to facilitate comprehensive care and referrals.
• Participate in collaborative case planning and case review meetings.
• Deliver case management activities including supervised visitation, parent aide support, and crisis intervention.
• Provide transportation and advocacy/linkage to services as needed.
• Complete in-home visits to monitor progress and support families.
