Qualifications

Responsibilities

• Coordinate with community providers to facilitate comprehensive care and referrals.
• Participate in collaborative case planning and case review meetings as requested.
• Deliver case management activities including supervised visitation, parent aide support, and transportation.
• Maintain documentation of services rendered, including case notes and service logs.
• Provide in-home visits to monitor progress and offer ongoing support for families.
