Job summary

Health Connect America is seeking an Independent Contractor-Family Support Services to improve family functioning and child well-being. Responsibilities include coordinating with community providers, participating in case planning, and delivering case management activities such as supervising visits and parent aide support.

The role involves in-home visits, transportation coordination, and ongoing support for families involved with DFCS, with documentation aligned to billing standards.

Qualifications

• Bachelor’s degree in a human services-related field (e.g., Social Work, Psychology, Counseling, Criminal Justice)
• or High School Diploma/GED with at least 5 years of verifiable experience in the field
• Strong understanding of family systems, trauma-informed care, and community resources

Responsibilities

• Coordinating with community providers to facilitate comprehensive care and referrals for families
• Participating in collaborative case planning and case review meetings, when requested
• Delivering case management activities such as supervision and transportation
• Providing in-home visits to monitor progress and provide ongoing support for families
• Offering parenting education and modeling positive parenting strategies
• Coordinating with external professionals to ensure service alignment
• Maintaining documentation of services rendered in accordance with agency expectations
