Job summary

Community Health and Wellness LLC is seeking Independent Contract therapists to deliver one-on-one therapy in Damon, TX, for individuals with disabilities. You will create tailored programs for conditions such as TBI, PTSD, Autism, Cerebral Palsy, and spinal injuries, and provide evaluations and ongoing services.

Responsibilities include home visits, client transport, HIPAA compliance, and weekly/quarterly documentation using Word, Excel, and Adobe Reader.

Qualifications

Must hold a Texas license in psychology, social work, counseling, or BCBA/behavior analyst.
Provisional licenses accepted per Texas Occupations Code.
Must have auto liability insurance (100k/300k) and professional liability insurance.
Valid driver’s license and transportation to client homes are required.
Access to a private laptop/desktop for HIPAA compliance.

Responsibilities

Travel to current or prospective clients' homes.
Be able and willing to transport client(s) during visits.
Have reliable transportation with valid driver’s license and auto liability insurance (100k/300k).
Communicate with health care professionals, clients, families, and the public verbally and in writing.
Use Word, Excel, and Adobe Reader to track goals weekly.
Implement patient care plans in home and community settings.
Work independently and manage your own time and client schedule.
