Job summary

Vistability is seeking a Tailored Day Services Specialist in Contra Costa County. The role involves providing one-on-one support to adults with developmental disabilities in workplaces, schools, and community settings.

You will focus on building independence and preparing participants for work or school. You will guide participants in setting goals, coaching in daily living skills, and documenting progress.

Qualifications

• Bachelor’s degree or related field or equivalent experience.
• At least two years of experience supporting people with disabilities.
• Able to travel to various service locations within Contra Costa County.
• Strong communication, organization, and teamwork skills.
• Proficient with Microsoft Word and basic computer programs.
• Fluent in spoken and written English.

Responsibilities

• Provide one-on-one customized support to adults with developmental disabilities.
• Help participants build skills for independence, work, and community involvement.
• Assist participants in setting and achieving academic and personal goals.
• Coach in social and daily living skills and encourage self-advocacy.
• Keep accurate notes and progress reports.
• Communicate regularly with participants, families, and partners.
• Assist with transportation to activities as needed.
• Follow safety and program policies.
