What We’re Looking For:
The In Reach Coach handles Diversion and In Reach activities for the TCL Special Healthcare Population with Serious and Persistent Mental Illness (SPMI), residing in settings such as Adult Care Homes, State Hospitals, Correctional Institutions, and community environments. Responsibilities include providing education about community resources, services, supports, and TCL permanent supportive housing. The role ensures members receive comprehensive, whole-person care addressing physical health, behavioral health, and social determinants of health to promote positive outcomes and community integration.

Qualifications:
Education: High School diploma or GED
Experience: At least 1 year of lived experience with a behavioral health condition and/or substance use disorder. Lived experience is defined as living in recovery from mental illness and/or substance use disorder.
Preferred Experience: North Carolina Certified Peer Support Specialist
License/Certification: Must be a North Carolina Certified Peer Support Specialist or obtain certification within six months of employment.
Applicants must meet NCCPSS Program eligibility requirements:
Preferred: Currently certified as a Peer Support Specialist
Other Requirements: Valid driver’s license
Remote Work Region: Must reside within 75 miles of Trillium’s Mid State region: (Anson, Guilford, Montgomery, Randolph, and Richmond counties)
