What We’re Looking For
The In Reach Coach is responsible for Diversion and In Reach activities for the TCL Special Healthcare Population with Serious and Persistent Mental Illness (SPMI) residing in Adult Care Homes, State Hospitals, Correctional Institutions, and various community settings by providing education about community resources, services, supports and TCL permanent supportive housing. The In Reach Coach ensures members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health/unmet health resource needs that helps to establish positive outcomes for members and facilitates community integration .
On a Typical Day, You Might
- Partner with Diversion staff, Transition Coordinators, and Next Step Rehousing Team to facilitate transition planning and progression through the TCL process.
- Collaborate with Trillium Integrated Care Nurse, OT, COTA and Clinician to identify complex care needs and clinical service linkage needs.
- Provide education and support to members and guardians
- Assist members/legally responsible person in choosing service provider, direct the planning process/plan development, and navigate the healthcare system.

Qualifications
Education: High School or GED
Experience: Be a person with (1) year lived experience with a behavioral health condition and/or substance use.
- Lived Experience is defined as living in recovery with mental illness and/or substance use disorder.
Preferred Experience: (if applicable)
North Carolina Certified Peer Support Specialist
License/Certification: Required to be a North Carolina Certified Peer Support Specialist, which may be attained within six (6) of commencing employment with Trillium.
The NCCPSS Program requires all applicants for certification to meet the following Program eligibility requirements:
- Must be 18 years or older
- Have lived experiences and in recovery from a serious mental illness and/or substance use disorder
- Must be in recovery for at least ONE year
- Have a high school diploma or equivalent
Preferred: (if applicable)
Currently Certified Peer Support Specialist Certification
Must have a valid driver’s license
Remote in Trillium’s Mid State region: Requires you to live within 75 miles of Trillium’s Mid State region: (Anson, Guilford, Montgomery, Randolph, and Richmond counties)
