Job summary

Total Life, Inc. is seeking nurse practitioners to lead longevity visits in the Dallas-Fort Worth area at partner senior living communities.

This in-person contract role starts with an educational talk for residents, followed by 45–60 minute visits focused on comprehensive geriatric assessment and medication review. You’ll be trained by our Chief Medical Officer and supported as you schedule and conduct site days, with travel to communities and Medicare documentation in our EHR.

Qualifications

• Active, unrestricted Texas APRN license with prescriptive authority.
• Current national NP board certification.
• Active DEA registration.
• Experience caring for adults 65+ and Medicare documentation.

Responsibilities

• Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment: what matters most to the resident, medication review and deprescribing, cognition, mood, sleep, nutrition, mobility, fall risk, daily function and advance care planning.
• Evaluate eligible residents for weight management, including GLP-1 therapy where clinically appropriate.
• Identify residents who would benefit from follow-up care, such as a fuller cognitive review and ongoing medical follow-up, psychiatry or therapy, and coordinate with their primary care provider.
• Document every encounter accurately and completely, to Medicare standards, in our EHR.
• Represent Total Life professionally with residents, families and community staff.
• Travel to assigned senior living communities in the Dallas-Fort Worth area.
