Job summary

AVIR at Del Sol in El Paso, TX is seeking compassionate Physical Therapists (PTs) to join our in-house subacute rehabilitation team. We offer Full-Time and Per Diem positions in a skilled nursing setting, with opportunities to collaborate across disciplines.

The role focuses on evaluating residents, designing individualized PT plans, and promoting mobility and independence while ensuring comprehensive documentation and patient-centered care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Texas Physical Therapist license in good standing.
• Experience in skilled nursing, subacute rehab, or geriatrics is preferred but not required.
• Strong clinical, communication, organizational, and documentation skills.
• Ability to work effectively as part of an interdisciplinary team.
• Passion for providing compassionate, resident-centered care.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve mobility, balance, strength, endurance, and functional independence.
• Monitor resident progress and modify treatment plans as clinically appropriate.
• Supervise and collaborate with Physical Therapist Assistants.
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies.
• Accurately document evaluations, treatment sessions, progress notes, and discharge summaries.
• Communicate resident progress and clinical recommendations with the interdisciplinary care team.
• Collaborate with physicians, nursing staff, and rehabilitation team members to provide coordinated, high-quality care.
• Participate in interdisciplinary team meetings and discharge planning.
