Avir at Tierra Este is seeking a Full-Time Physical Therapist to join our in-house rehabilitation team in East El Paso. You will conduct evaluations, create customized plans, and deliver evidence-based therapy to residents in a supportive setting.

Qualifications

• Active Texas Physical Therapist license in good standing.
• Experience in SNF or long-term care preferred.
• Experience with Net Health documentation is a plus.
• Strong communication skills and ability to work with diverse populations.

Responsabilités

• Evaluate residents’ physical conditions and rehabilitation needs.
• Develop and implement individualized treatment plans using evidence-based techniques.
• Provide interventions to improve mobility, strength, balance, and functional independence.
• Monitor progress and update care plans with the IDT.
• Educate residents and caregivers on exercises, safety, and mobility strategies.
• Document accurately per facility and regulatory requirements.
• Attend team meetings and participate in interdisciplinary coordination.
• Adhere to safety, compliance, and regulatory standards.
